Medical Records Requests

Please direct all medical records requests and all future requests to our online platform.

Instructions

1. Sign up for an account to submit records by clicking this link: https://app.chartrequest.com/6446. Confirm your email account by clicking the link sent to your email after signing up.
Once inside your account, click “Make a New Request” and select “Matthew C. Frankel, MD, FACP” from the drop-down menu.
Fill out your request form and upload your signed authorization form. Click “Send to Provider” to submit your request.
You will receive an email once your records are available online. Log in and, if applicable, pay anytime to download your records.

Once you’ve created an account, you can submit future records requests in under 3 minutes- no more waiting on hold or lost faxes! Thanks for your help, and feel free to email support@chartrequest.com if you have any questions!
